A retired Army engineer friend sent this butt hurt documentation form. It's set up for the Army but any service, organization, or agency would work just as well. Couldn't get it all in one shot so the first is the top and the second is the bottom of the form...

They should have handed these out with the stress cards they used in the 90's.

I've heard rumors of those but I never met anyone who has actually seen one. I could have used a few myself whist I was DO of the 9th Airevac Squadron.

Those afraid of the universe as it really is, those who pretend to nonexistent knowledge and envision a cosmos centered on human beings will prefer the fleeting comforts of superstion.  They avoid rather than confront the world.  But those with the courage to explore the weave and structure of the cosmos even where it differs profoundly from their wishes and prejudices, will penetrate its deepest mysteries.-Carl Sagan

That's great!  Gonna print out a few or a hundred ... I could easily hand out 4 or 5 of these every single work day.
Thanks Ken!

I am not concerned about what you think as I can tell you don't do it often.

I used to be a people person, but then people ruined that for me.

I saw the recruits using them while I was at the NCO Academy at Ft. Knox.  Luckily the program was scrapped after just a short time.  Produced lousy soldiers with little respect that couldn't follow instructions.  It was almost as good an idea as co-ed boot camp and the period of time you could only make a soldier do 10 push ups at a time.

Here is a great Geico commercial that fits the subject.  http://www.youtube.com/watch?v=XfmVBmDKLZI
